Lifestyle is a unique new health plan rider designed to improve the health and quality of life of its members. Lifestyle gives members $250 to use for any of the following services:

The following exclusions apply:

• All products and/or services not listed above.
• Fitness classes not included with the cost of a gym membership unless otherwise listed above.
• Diet classes, centers and weight loss programs
• Day Care/Child Care
• Parties and/or special events
• Merchandise or gift certificates
• Hair and color, and make-up services excluded under Spa Services

* Spa Services – only therapeutic Services are covered under this category. A therapeutic service is defined as a form of therapy intended to heal injury, relieve psychological stress, manage/relieve pain, improve circulation, rejuvenate the body, improve health vitality, care for the skin and alleviate tension and fatigue.  Please note cellulite treatments, aromatherapy pedicures/manicures, hair removal and scalp treatments are considered cosmetic, and are NOT covered.
