Prism Quality EAP Summary of Services

An EAP implementation process, with input from organization representatives to ensure successful orientation to the EAP Program.
24-hour toll free access to services.
Bilingual services.
Face-to-face EAP sessions to assess the individual's problem, determine an appropriate level of intervention and as appropriate, provide short-term, solution-focused counseling.
EAP trainings to be provided at the employee's location or a mutually agreed upon location.
Management and Supervisory consultation as needed.
Crisis intervention and coordination of a face-to-face evaluation with a mental health professional at the worksite to assess the need of treatment services for employees who are threatening to harm themselves or someone else.
Critical Incident Stress Debriefing (or Trauma Response). This service is a group activity provided at the location of the business impacted by the incident. A CISD is generally utilized to address events that produce significant emotional reactions in individuals now or in the future. Examples of such events include accidents, employee death, violence and natural disasters. Critical incidents at the workplace can severely affect employee morale and performance.
Fitness & Nutrition services, including educational seminars, fitness passes to our State-of-the-Art fitness facilities and a "Fitness Report Card" completed by licensed exercise physiologists.
Occupational injury prevention & management services.
Referrals to treatment providers and community resources including a full range of chemical dependency and mental health services.
Mediation/Conflict Resolution services.
Referrals to legal and financial services.
Referrals to childcare services.
Referrals to eldercare services.
Referrals to vocational counseling to assist displaced employees.
Follow up for a minimum of 30 days for each employee who utilizes the EAP .
Marketing materials including wallet cards, announcements, posters and other appropriate information at inception and in reasonable quantities throughout the year.
Surveys offered to each participant who utilizes the EAP to determine level of satisfaction and identify areas of improvement.
Quarterly and annual utilization reporting.
